CWHA provides a roster of health, immigration
and social support programs that aim to improve the well being
of individuals, to strengthen families and empower communities.
These programs are uniquely designed to provide comprehensive,
integrated, culturally appropriate and coordinated “one-stop”
services.
CORE PROGRAMS

Maternal
and Child Health:
|
Community Mom's Program - Provides community health education workshops, supports services and home visits to pregnant and parenting moms with children 0-2 years old. The Mom's program also does screening and referrals for perinatal depression. |

CHP/FHP
–Child Health Plus/ Medicaid/WIC Facilitated
Enrollment Initiative: Provides facilitated
enrollment into Child Health Plus or Medicaid for
children under 19 years old. It also facilitates
enrollment of adults into Family Health Plus, who
are not qualified for Medicaid. |

Women,
Infants and Children (WIC) Nutrition Program: Provides
nutrition counseling and food coupons to infants,
children and pregnant/parenting women with children
5 years of age and younger. |

Comprehensive
Prenatal-Perinatal Services Network of Brooklyn
(CPPSN): Through network involvement, members
strategically plan goals focused to contribute to
the reduction of low birth-weight and infant mortality
rates by providing leadership that will strengthen
collaboration among prenatal/perinatal providers,
facilitate improved service delivery, and collect
utilization and outcome data. |

Infant Mortality Reduction Inititiative: In view of the devastating impact on the Caribbean-American community resulting from the disproportionately high rate of infant mortality, Caribbean Women's Health Assocation, Inc (CWHA) has launched an aggressive campaign to fight infant mortality. The campaign is designed to mobilize community resources, to improve pregnancy outcomes through community education, and remove the barriers towards providing access to perinatal care. |

Education and Case Management Program:

AIDS
Education and Case Management Program: Provides
community outreach, education and case management
services to HIV infected or affected individuals.
The program also provides technical support, assistance
and training to other providers of AIDS/HIV services.

Educational Case Management Program: Provides
home visits, education, case management and referral

Teen
Abstinence Educational Program: The CWHA “PHAT”
(Promoting Health and Abstinence Education Among
Teens) program provides peer educators and parent
advocates to promote the delay of sexual activity
and foster healthy development for adolescents.
The program works with adolescents ages 10-19 with
emphasis on ages 10-14. |

Family
Unification: Provides legal counseling, support
and educational services designed to foster family
unification and help families access needed services,
adjust to their environment, and resolve immigration
problems. |

Citizenship:
Assists immigrants in becoming American Citizens.

Legal
Defense: Provides legal services representing
clients in Immigration Court. |
|
Immigrant
Rights: Initiates, endorses, develops, supports
and provides programs designed to protect the rights
of immigrants. |

CWHA operates
the Caribbean American Family Health Center (CAFHC)
in partnership with Lutheran Medical Center and
Sunset Park Family Health Center Network. |

OB/GYN, Pediatrics, Internal Medicine and Family

International:
Conducts and participates in international
conferences and field tours to study public health systems,
and foster technology transfer and cross-cultural education.

Cross-Cultural
Health: Conducts interdisciplinary studies and
activities addressing cross-cultural health issues. |

Volunteer/Internship:
Offers individuals an opportunity to gain professional
experience and make invaluable community service contributions.

Conference/Seminars:
Addresses health, immigration, and socio-economic issues.

Youth
Services: Designed to provide adolescents
with basic work experience and skills development. |

Special
Events: Sponsors, “And Still They Rise,”
and “Laureates of Health Care” benefit receptions
to recognize outstanding leaders. |

Advocacy:
Conducts research, publishes reports, operates a Speakers
Bureau, and testifies at public hearings. |

Management
Support and Technical Assistance: Provides management
support and technical assistance to providers serving
ethnic, immigrant and culturally diverse populations.

the following: its multi-cultural and multi-lingual programs
and personnel; its demonstrated strong fiscal management,
reporting systems, and contract compliance with funding sources;
its well developed infrastructure for effective administration
and supervision of programs; its ability to plan and effectively
administer programs with measurable outcomes; and its solid
record of providing successful outreach, education and counseling
services in a culturally sensitive manner to low income and
immigrant/ethnic communities in New York City.
